dgp 1446838d71 To use a Tcl_Command token [aka (Command *)] for epoch checking, we must not
permit it to be freed while we hold it or else it could be mistaken for another
token allocated later that just happens to reside at the same address.

(Command *) preservation machinery already exists, just need to use it.

An extension facing the same problem might have to rely on command delete
traces.

Earlier revisions used (Namespace *) lifetime to achieve the same
results, but that's really an indirect (possibly non-robust) path to achieving
the proper goal.

Valgrind is happy now.

dgp 76f6131b81 The "ensembleCommand" Tcl_ObjType has no need for an UpdateString routine.

dgp 1438a1b7f7 Remove the nsPtr field from the EnsembleCmdRep struct. Cannot see any
justification for keeping it (can get via token), and test suite doesn't care.

dgp 024bab4116 Bug fix. Have to arrange to only close a catch once. After the space
has been returned to placeholder values, closing with them as data leads
to memory corruption.

There's probably a better fix available because the error here feels like
it's rooted somewhere else, having us continue to check values we ought to
know have already been closed.

Note that the pipe buffering semantics can vary at the operating system level
substantially; it is not safe to assume that a write performed on the output

fundamental difference and Tcl cannot conceal it. The overall stream semantics
