Add CODEOWNERS, MAINTAINERS.md, and issue auto-acknowledgement

Auto-requests review from the code owner, documents additional maintainers
who are not auto-requested, and posts a thank-you comment linking to
CONTRIBUTING on new issues from non-maintainers.
